Musculoskeletal (MSK) physiotherapy treats the muscles, joints, ligaments, tendons and nerves that let you move. Most people will have back, neck or joint pain at some point, and physiotherapy is one of the most effective, evidence-based ways to recover and stop it coming back.

What is MSK physiotherapy?
Musculoskeletal (MSK) physiotherapy is the assessment and treatment of problems affecting your muscles, joints, ligaments, tendons, nerves and spine. It is the most common reason people see a physiotherapist, and it covers everything from a stiff neck after a long week at a desk to persistent low back pain, arthritic knees or a shoulder that won't lift above your head.
Physiotherapy is recommended by NICE as a first-line treatment for most back, neck and joint problems. Rather than masking symptoms, it aims to restore normal movement, rebuild strength and give you the knowledge to manage your condition yourself.
Common causes of back, neck and joint pain
Most MSK pain is not caused by serious damage. It is usually the result of a combination of factors: sudden or unfamiliar activity, long periods in one position, reduced strength or fitness, stress, poor sleep and previous injuries. Desk work, driving, lifting children, manual jobs and returning to exercise after a break are all common triggers we see in patients from Slough and Ealing.
Age-related changes such as osteoarthritis are also very common, but scans often show changes in people who have no pain at all. What matters most is how well you move and how strong and confident you feel, which is exactly what physiotherapy improves.
How physiotherapy helps
Your physiotherapist will first find out what is driving your symptoms. We look at how you move, test strength and flexibility, check nerve function where relevant, and screen for anything that needs medical review. You will leave your first appointment with a clear explanation and a plan.
Treatment usually combines hands-on techniques such as joint mobilisation and soft-tissue release to ease pain in the short term, with a progressive exercise programme to fix the underlying problem. We also give practical advice on posture, work set-up, sleep positions and how to stay active without making things worse.
Research consistently shows that people who stay active and follow a guided exercise plan recover faster and have fewer flare-ups than those who rest completely.
What you can do at home
Keep moving within comfortable limits: gentle walking and regular changes of position help far more than bed rest. Heat can ease muscle tension, and simple pain relief recommended by your pharmacist may help you stay active. Avoid sitting for long periods; set a reminder to stand and move every 30 to 45 minutes.
If pain is not improving after a week or two, or keeps returning, it is worth getting an assessment so small problems don't become long-term ones.
When to seek urgent medical help
Contact NHS 111, your GP or A&E straight away if back pain comes with numbness around the genitals or bottom, new difficulty passing urine or controlling your bowels, weakness in both legs, unexplained weight loss, fever, or follows a significant fall or accident. These are rare, but need urgent medical review.

Specialist pain care and injections
Most back, neck and joint problems improve with physiotherapy. If your pain is not settling as expected, your physiotherapist can arrange a review with our consultant in pain medicine, Dr Sanjay Kuravinakop, who can discuss further options with you.
Where clinically appropriate, this may include image-guided injections. An injection is only considered after a full assessment and a clear discussion of the benefits and risks, and it is always combined with a rehabilitation plan.
